New Provider Tax ID Requirement Effective January 1, 2019 Posted: November 9, 2018

ProviDRs Care is asking all participating providers to ensure all necessary practice information on file with ProviDRs Care is up to date, particularly Tax Identification Numbers (TINs). Effective Jan. 1, 2019, ProviDRs Care will reject claims submitted by contracted providers whose TIN(s) are not on file until you complete the update.

To minimize claims disruption, we at ProviDRs Care are currently reviewing claims to determine who this will impact and are reaching out to these providers prior to the deadline.

To verify information currently on file with ProviDRs Care, please call Provider Relations at 800-801-9772, option 4. If you need to update practice information, please complete the Provider or Practice Updates and Changes form located at https://providrscare.net/wp-content/uploads/2015/11/Provider-Changes.pdf. Completed forms can be returned by following the instructions at the bottom of the form.
