Important Notice — Amendment to Provider Agreements

We are updating the terms of our Physician, Hospital, and Supplier Agreements and wanted to share the details with you.

Updates will go into effect for Group renewals on or after October 1, 2021, for the following documents:

• Physician Provider Agreement
• Hospital Provider Agreement
• Supplier Agreement

The following is a summary of the important changes, for your convenience.

Network Access Requirements

• Group is required to adhere to network access requirements, including, but not limited to administrative procedures and benefit differential standards.
