We are excited to announce that as of November 1, 2015 we are utilizing and have access to the CAQH data bank. Our web site has been updated to accept the applicable CAQH number on our online application. When accessing our web site at www.ProviDRsCare.net, you will see we no longer need the CAQH application. All that is needed at this time is to provider’s CAQH number.
