Reminder: ProviDRs Care Is Requiring Tax ID Information Update

In November, ProviDRs Care asked all participating providers to ensure all necessary practice information on file with ProviDRs Care is up to date, particularly Tax Identification Numbers (TINs). Effective Jan. 1, 2019, ProviDRs Care will reject claims submitted by contracted providers whose TIN(s) are not on file until you complete the update.

If you need to update practice information, please complete the Provider or Practice Updates and Changes form located at https://providrscare.net/wp-content/uploads/2015/11/Provider-Changes.pdf. Completed forms can be returned by following the instructions at the bottom of the form.
