NOTICE OF UPCOMING PROVIDER DATA AUDIT

On Oct. 1, ProviDRs Care will perform an annual data audit of participating network provider’s basic information to ensure our systems are up to date. This includes demographics and contact and billing information.

If you receive a Provider Confirmation form on Oct. 1, review the form to:

• Confirm we have the correct information on file, or
• Add changes

If we do not receive Provider Confirmation form, your claims may be rejected, so it is important that you respond as soon as possible.
