REMINDER NOTICE OF UPCOMING PROVIDER DATA AUDIT

On Oct. 1, ProviDRs Care will conduct an annual Provider Data Audit to ensure our systems and Provider Directory is up to date.  This includes demographics and contact and billing information.  Providers selected for the audit will receive a Provider Data Confirmation form.

If you receive a Provider Data Confirmation form, review the form to:

• Confirm we have the correct information on file, or
• Add changes

Forms are required to be returned within five business days of date on the form.

Please return completed forms to ProviDRs Care via:

If we do not receive a completed Provider Data Confirmation form, your claims will be rejected, so it is important that you respond as soon as possible.
